ACP's microlearning series “Decision-Making for Patients with Multiple Chronic Conditions” is a practical, easy-to-use resource designed to support internal medicine physicians in delivering care that aligns with what matters most to their older adult patients who are managing multiple conditions.

This 10-part series offers actionable strategies for identifying patient goals, navigating complex tradeoffs, and tailoring care decisions to individual needs. The series follows a real-world case featuring Mrs. B, a 77-year-old woman managing multiple chronic conditions. Each activity lasts 10 minutes.

ACP members receive free access to the series, along with the opportunity to take MOC quizzes and claim CME/MOC credits.
