Decision making with patients who have multiple chronic conditions is complicated, so ACP's new Patient Priorities Care online training series offers practical steps for physicians and care teams to learn how to help patients identify their health priorities and then to align care to them. Clear priorities that are understood by patients, their caregivers, and their clinicians can drive care that is less burdensome, better integrated, and consistent with what matters most to patients.
The online curriculum consists of three modules:
- Introduction to Patient Priorities Care
- Identifying Health Priorities
- Aligning Decisions and Care with Patients' Health Priorities
Free CME/MOC is available to those who complete each module in the curriculum. The training activities can be started today.
