ACP's Patient and Interprofessional Partnership Initiative offers new toolkits

The General Team Care Toolkit shares best practices and real-life examples of team-based clinical care models, while the Engaging Informal Caregivers Toolkit helps physicians partner with caregivers.


ACP's Patient and Interprofessional Partnership Initiative works to promote high-quality education that incorporates interprofessional, interdisciplinary, and patient perspectives and promotes partnership with all members of the health care team. This initiative has recently developed patient-centered, interprofessional education resources for internists, their patients, and their clinical teams in the form of two new educational toolkits.

The General Team Care Toolkit shares best practices and real-life examples of successful, team-based clinical care models that include internal medicine physicians working with nurse practitioners, physician assistants, and other members of the health care team.

The Engaging Informal Caregivers Toolkit shares training, best practices, and resources to help physicians partner with caregivers to improve health outcomes.
