ACP Practice Advisor® has released an updated module on managing diabetes and improving care delivery for patients with diabetes.
The “Managing Diabetes Mellitus” module offers resources for clinicians to help identify key practice-level interventions to optimize the delivery of care to patients with diabetes, motivate patients with diabetes to improve their self-management skills, and help practices deliver optimal diabetes care.
Successful completion of this activity enables the participant to earn 20 CME and 20 MOC Practice Assessment points.
