ACP's Center for Patient Partnership in Healthcare releases new blood clot resources

New ACP resources include a web-based assessment for determining personal blood clot risk, a self-management guidebook, patient information sheets, and an office poster.


ACP has developed patient-centered, free, educational resources about blood clot prevention, risk factors, and symptoms. Blood clots claim over 100,000 lives annually in the U.S. Many at-risk patients often do not understand or are not aware of blood clot symptoms or their risks for developing blood clots. Resources include:

  • a web-based assessment for determining personal blood clot risk (after answering a series of simple questions, patients can print out a final summary of their risk for blood clots to use for patient-physician discussion);
  • a “What You Should Know About Blood Clots” self-management guidebook (in both English and Spanish);
  • Patient FACTS information sheets about deep venous thrombosis and pulmonary embolism (in both English and Spanish); and
  • an office poster that encourages patients to “Go With the Flow” by knowing their risks for blood clots and speaking to their doctor.
